1.Intro
In this intro you will learn to create a hearth with a 3D effect. This may be a bit too simple, so we are going
to ad a cut and some blood. After that, we will take care of the wound. We will do all this using only the standard Photoshop CS features, no plug-ins, no
downloads. Ok, let’s get started
2. The Heart
500 x 500 is a good size for the canvas because it is large enough to have details but small enough to fit on the screen and to not to
kill your
processor and steal your RAM. Ok, now grab the Custom Shapes tool (U) and draw you heart shape.

 
Duplicate the heart layer and make it a bit smaller, center it and change the color to a light
yellow.
Create a layer mask and grab the Gradient Fill tool. Create the gradient seen in the layer mask on the Layers Palette
 
Now it’s time to add some details. Duplicate the new layer twice and set the original highlight layer to 50%
opacity. For the two new layers use the Gradient Tool (on radial) to create little highlights on each bump of the heart (keep the gradient length shorter
rather than longer.) Keep these layers on 100%. Duplicate the highlight layer (any of the 3) and change the color to black, set the layer mode to overlay and
then change the gradient in the mask to feature the point of the heart as shown.

You should get something like this:

Now pick the brush tool, and select a soft round brush (20 – 27 pixels).

Set the color to dark red and the pressure to 5- 10%. Brush the top and the margins of the heart to get a 3D effect.

Merge all the heart layers into one, and you are ready for the next step.
3. The cut
This heart is going to have a hard time from now because we are going to add a nasty cut. First,
create a new layer. Use the lasso tool to make a selection that looks like a
cut. Your hand is shaky? Good! The cut will look more real. Fill the selection
with black.

Ok, let’s make it look more real: Choose the brush tool, and select a soft round brush,
with a small diameter (depending on your cut). Set the color to dark red and the
pressure to 5- 10%. Magnify the picture and add a contour to the cut. After
that, we are ready to add the blood.

4. The blood
Create a new layer for the blood. Select the Custom Shapes Tool
again and choose the blob shape with black color. Create your shape and than
drag the layer below the heart layer.

Duplicate the blood layer and change the shape color to
dark red. Create a layer mask and grab the Gradient Fill tool. Create the
gradient seen in the layer mask on the Layers Palette.
 
 
After this use the Rectangular Marquee tool to help you draw a straight line from the wound. Now we got some blood pouring from the cut.

Things are getting serious, better take care of that.
5. First Aid
First, create a new layer. Now use the Polygonal Lasso Tool to define a selection across the cut.

Fill the selection with a very light yellow, then go to the Filters menu and choose Texture->Patchwork. The result should be something like this:

Now pick the brush tool again and play with this selection to make it look more real… and this is it.
